antibioticsI’ m sure all of us ever consumed antibiotics, but how to get or to prescribed rational antibiotics ? Prophylactic antibiotics are used to prevent infection from contamination of a wound. They should not begiven to prevent other postoperative infections (e.g.chest infection).

Audit has shown that, not only are the wrong antibiotics often chosen, but, more importantly, the timing is often incorrect. Case–control studies have failed to show conclusively that one antibiotic is superior for prophylaxis. The choice of antibiotic depends on the likely pathogens that occur at various sites. Antibiotics should be given intravenously at induction of anaesthetic to facilitate high concentrations in the operation site at the time when contamination occurs. A number of well-controlled trials have advised single-dose prophylaxis over multiple doses.

In my country, many older people consumed corticosteroids. Its really a god’s drug, but do they know it can induced osteoporosis. Many postmenopausal women and older men receiving corticosteroids are probably at the greatest risk of rapid bone loss and consequent vertebral fracture and should be strongly considered for preventive measures. In younger individuals receiving corticosteroids, the decision to use osteoporosis prevention is less straightforward and will depend upon a number of factors including baseline BMD, anticipated dose and duration of corticosteroids and other risk factors. Based upon available evidence the rank order of choice for prophylaxis would be a bisphosphonate followed by an active vitamin D metabolite or HRT.

Although calcium alone appears unable to prevent rapid bone loss in patients starting corticosteroids, it should be considered in those patients with normal BMD values. If an active vitamin D metabolite is used, calcium supplementation should be avoided unless dietary calcium intake is low. The evidence for hormone replacement is limited, but HRT should be considered if hypogonadism is present. Addition of a bisphosphonate in postmenopausal patients already taking estrogen replacement therapy may enhance the bone density response. In patients receiving chronic low dose corticosteroids, treatment with calcium and vitamin D may help to prevent further bone loss.

CVSFor me, this is the first time I ever heard about this terms : Computer vision syndrome (CVS). Based on Wikipedia CVS is a temporary condition resulting from focusing the eyes on a computer display for protracted, uninterrupted periods of time. Some symptoms of CVS include headaches, blurred vision, neck pain, fatigue, eye strain, dry, irritated eyes, and difficulty refocusing the eyes. These symptoms can be further aggravated by improper lighting conditions (ie. bright overhead lighting or glare) or air moving past the eyes (e.g. overhead vents, direct air from a fan). CVS has not been proven to cause any permanent damage to the eye.

These problems are more noticeable with computer tasks than other near work because letters on the screen are formed by tiny dots called pixels, rather than a solid image. This causes the eye to work a bit harder to keep the images in focus.
